Office visits and any medically necessary testing,
depending on the exam results, may be billed to either
the patient, a routine vision
insurance, or a major medical insurance. This is similar to
the concept of a "sick" visit verses a "wellness" checkup.
Insurance benefits
that are quoted are not a guarantee of payment. The
patient will be responsible to pay for any co-pays,
deductibles, or rejected claims.
Members of an HMO are responsible for obtaining a referral for
their visit if necessary and agree to pay for any services for
which a
referral was not obtained.
Payment for examinations is due at time of service. For all
orders of eyewear, contact lenses, or therapy services: full
payment is due at
time of order. All transactions are final and there are no
refunds. There will be a service charge on all returned
checks.
